Overview
Desmoglein 3 (Dsg3) is a cell-adhesion protein found mainly in the deeper layers of the epidermis and mucous membranes. This test detects IgG autoantibodies against Dsg3 in serum and is primarily used as an aid in the diagnosis and monitoring of pemphigus vulgaris (PV).
Clinical Indications
The test may be recommended for patients with:
- Unexplained mucosal or skin blisters and erosions
- Suspected pemphigus vulgaris
- Monitoring of diagnosed pemphigus vulgaris
- Assessment of disease activity or response to treatment
Interpretation

Positive: Anti-Dsg3 antibodies are detected and may support a diagnosis of pemphigus vulgaris when correlated with clinical findings.

Negative: Anti-Dsg3 antibodies are not detected; however, a negative result does not completely exclude pemphigus.
- Antibody levels can correlate with disease activity in many patients, but results should be interpreted together with clinical examination and other investigations.
Important Note
This is an autoantibody test and should not be interpreted in isolation. Histopathology and direct/indirect immunofluorescence may also be used when evaluating suspected autoimmune blistering disorders.
Patient Preparation: Usually no special preparation is required.
Sample: Blood/serum sample.
