What is the test?
Desmoglein I (Dsg1) is a protein that helps maintain adhesion between skin cells. Desmoglein I Antibody testing detects autoantibodies directed against Dsg1 in the blood.
Clinical Significance
The test is primarily useful in the evaluation of autoimmune blistering skin disorders, particularly:
- Pemphigus foliaceus
- Pemphigus vulgaris, especially when skin involvement is prominent
- Other suspected autoimmune pemphigus disorders
When is it advised?
It may be recommended when a patient has:
- Superficial or widespread skin blisters
- Erosions, crusting or peeling of the skin
- Suspected autoimmune blistering disease
- A need to support diagnosis or monitor disease activity

Interpretation
Positive Dsg1 antibodies: May support an autoimmune pemphigus disorder, particularly pemphigus foliaceus, when correlated with clinical findings and other investigations.
Negative Dsg1 antibodies: Makes Dsg1-associated disease less likely but does not completely exclude an autoimmune blistering disorder.
Note: Results should be interpreted along with clinical examination, histopathology, direct immunofluorescence, and other relevant antibody tests such as Desmoglein 3 antibody, when indicated.
